My group went in there yesterday for the first time, tried T2 and had no problem activating the Caerygs (dealing with them is another matter...).
-We wiped on Coldbear once.
-We did the challenge on the second boss w/o wipes though we reset it once inadvertently with someone going up the stairs.
-Some people left instance to repair and we had to replace someone after a couple of wipes on the last boss.
There seems to be a combination of factors that may make challenges reset in new instances. For example, the 1st time I did NC Farm, I left the instance to retrait while we were trying to do the last boss (that was before we learned to read...). When I came back my challenge had reset and I was thus unable to complete it (the cauldron was down). Another member of the group left the instance at the same time to get pots but kept his challenge.

Each person that enters the zone has to progress the three challenge objectives, if anyone isn't flagged with the previous two being completed, they wont activate.
Which means, any of the 6 cannot leave the instance at any time, this means no repair/pots/ect or you quests will reset
No switching bodies, if you bring someone new in, they wont be flagged.
All 6 ppl need to be flagged for Coldbear and 4 Champs, before Drugoth with work properly.
Nobody in my group left the instance, and everybody was flagged correctly. Last fight still didn't work correctly.

The idea of challenge modes (or challenge quests that span multiple boss fights) resetting if you leave the instance is a very bad idea.
If someone in my group has to drop because of an emergency, I shouldn't have to choose between failing the challenge or summoning a replacement person to the instance.

We killed the 4 lieutenants.
The quest then updated to kill the 2 guardians.
We did reset after the 4th luetenant died, and just dps'd the giant hard and he died without bothering any lieutenant.
The guardians did not activate.
I guess we failed even though we followed the challenge exactly as it is written.
The quests need to reflect the actual state of the challenge,

I think the workaround is going to be killing every active mob that's up after the 4th LT dies and before killing the boss. If anyone does this and still gets the bug on boss 3, please post with details. Just working on a process of elimination.

We think we found the problem and for now, you can work around this by making sure you kill every add in the Osan fight, not just the lieutenants. We will fix this very shortly.
Sorry about that.
And for the record, you can totally bring people in and still complete challenge. That would be just mean!

I'd double check this. We purposefully didn't dps Osan at all, until every single Giant, Goblin, and 4 Champs where killed off. There was nothing left above us at all. We still got the normal waves of three adds, but everything was cleared with out a doubt.
We switched out a LM for a Capt on that fight.
After clearing with Capt, Tank left instance to go pick some flowers. *I'm not kidding-Thats how we roll*

Ok amlug need some clarification.
We killed EVERY single add in Osan fight, everything at the top was dead all his reinforcements were dead and we still didn't get to activate carog. But one thing that I did notice is that the adds in coldbear did not all activate and die.

We had the Caeryg refuse to activate today. We had reset Osan, but we'd made sure to kill all of the adds the second time. We did not get any wights during the Coldbear fight.

there is something else causing sporadic lack of activation. the most likely is still something to do with adds in the second boss fight.
i cant remember this was a whole day ago, but im pretty sure someone in group left the instance and we got the guardians as well.
we did reset the second boss.
cant remember if we killed all the adds or not, but im almost positive we did. so looks like its the adds to me.

But if you remember, some of the wights in the hallways prior to Coldbear didn't activate. I'd hate to think you'd lose the challenge because a few trash mobs didn't activate, but given what people are saying, it seems possible.

However, ppl who join mid-course will not get the challenge and the other instance quest, right? If this is the case, I hope there will be many altruistic people out there once the novelty wore off and most jewelry has been discovered/looted... since as I understand it they won't get Meds for helping out.

Did this last night, killed all the adds at Osan other than the waves of 3 that keep spawning infinitely. Caerogs did not activate. We reset the last boss to try to get the challenge to reset but no luck. On the way to coldbear we left one matron alive.
You really need to fix this, I want mah loots! And it is a really long and involved and tough instance which I like, but then to be slapped in the face with thx for spending an hour in this instance, now you will get jack, well its kind of like a...slap in the face.

Ran this with my kin last night. We wiped/reset on Osan 3 or 4 times while figuring out positioning. Killed all adds, all captains. Nothing alive when the boss died. When we went into the final boss room, the guardians didn't activate. The group wiped while I was attempting to open a ticket. On our next attempt, I opened a ticket, and a GM sent me a tell that the guardians had been activated. One of them started to move, then the group wiped. The GM closed the ticket, but said the guardians should activate normally on future attempts. Third attempt the guardians did not activate. Opened another ticket, same GM said he was activating them. Our group got into position to kite/kill them, then we watched as they activated and crumbled without entering combat or give us credit in the quest log.
After the guardians crumbled the GM told me there was nothing else he could do to help. We went ahead and cleared the boss. We never got our challenge medallions but we did get the tier 2 chest.
I /bugged this as soon as we were done. Long story short, it looks like the GMs can't help if the guardians don't activate.

All 3 people died by misadventure within 2 seconds of each other. I asked the people to check their combat logs to see what caused it and they said it was just typical rallying cry heals and RK hots then bam...dead. I posted this in the LT thread because similar people seem to have experienced the issue there (supposedly dying by misadventure when they shouldn't have) but I was told I should post in one of these cause a certain GM or dev might know the mechanic behind it.
This just seems like a really random spot for...misadventure. Could anyone explain how it happened? It was kinda funny, but really dumb. lol

Just encountered this bug tonight. I was hoping today's scheduled maitenance was going to cover this, especially since it's well documented and has had a blue post in the thread, but I guess not.

Ditto, ran this last night. Had a couple of deaths but got through it all.
Killed everything before Osan went down, and like others have said, guardians didn't activate.
Side note: Osan hits like a ICBM.

Had the Guards script for us tonight. So off the top of my head I'll just list off random things that we did during the instance. None of it is probably the cause of a successful clear but *shrug*
-Left several Big bears and bear cubs alive. When you go middle path to Coldbear to the right is a Bear and cub, to the left ic bear and cub, we just ran past them.
-Two sets of coldbear adds were left in Ice
-We had the second boss to under 10k before the fourth and last captain spawned.
-When the last Captain went down boss was at 4k morale, then we killed him off.
-There was one wave of small adds during the fourth and final captain-cleared them before Captain was finished off.
Great instance, awesome fights and really nice loot for casters/capts
