I wasn't me who said it but I think they were referring to settlement names, not area names in general.

I'm not so sure Rohan and Gondor were so empty as you make it sound. The amount of towns and cities in LotRO seem plausible for those two countries and the number of troops they were able to...

The "mix" is about 80% made up names and 20% from the lore, I'd say. The farther you get from The Shire the more lopsided that mix is.
