Preface:
So I help out my handicapped friend on LOTRO. I have five characters at lvl 140, and a ton of alts. He's handicapped enough that I have access to his account so I can fix things and make purchases for him (I've bought him several expansions, as he has very limited income). We play slow, but over the years, we've gotten to lvl 107. His main is a Lore Master ==> does lots of damage + he has a pet (he's not very good at fighting). We just got to Minas Tirith. TONS of quests there. It filled up my quest log, but not his... He has 60 slots, I have 50... ???
I did some research:
You start with 40
Each character can get 10 more for doing 400 deeds ==> we both have that
Account wide, you get 10 more for buying the Helm's Deep Premium Edition. ==> I didn't think either of us got this
Each character can get 10 more for 40 mithril. ==> we haven't bought this yet
Total Possible = 70 quest slots...
Hmm... I bought basic Helm's Deep back when it came out. I got my friend the Helm's Deep Expansion from the free summer giveaway (I would have bought it, but we were still in Rohan when they did that awesome giveaway)... Uhmm --was that the Premium Helm's Deep from the summer giveaway? That's the only thing I can imagine. I looked back on the LOTRO market and in the LOTRO store, and there's no way to buy the Helm's Deep Premium Expansion anymore. I didn't get the free offer because I already had it...
Active Quests
Characters start out with 40 active quest slots, meaning that they can accept up to 40 quests in their Quest Log at a time. Additional quest slots can be obtained as follows:
•VIP players get an additional 5 quest slots
•10 additional quest slots can be purchased with 40 Mithril Coins
•Completing 40 Deeds grants 1 additional quest slot up to a maximum of 10 quest slots from completing 400 Deeds
•Purchase of the Premium Edition of the Helm's Deep expansion granted 10 additional quest slots
My main is Level 134 and has 55 quest slots for the quest log: 40 (base) + 5 (VIP) + 10 (deeds). If you have 50 at Level 140 that's probably the same as me except you don't have an active VIP subscription. Your friend could have those 10 slots more than you because he did purchase Helm's Deep Premium, or purchased slots with Mithril Coins, or some combination of Mithril and a currently-active VIP subscription.
You might not be the only other person with access to his account credentials for making purchases.

No no... He is handicapped enough that I *have* to manage his account. No one in his family is into gaming. Mom doesn't give him a credit card. In game, I have to manage a lot of elements --handling his LI's for instance. He can't read the map, so I lead him around to the various quests. He'll let his inventory fill up, so he can't receive new rewards, so I have to manage that. Buying things on LOTRO store, etc...

But the ONLY possibility here is that the free Helm's Deep from the summer giveaway just happened to be the Premium Edition. But then if you bought the basic edition, you didn't get the giveaway...
